Helpful Hints When Referring a
Patient
We are now providers for B/C B/S Health
Options and Health Options Medicare. An authorization number is
required. To easily obtain an authorization number you will need
our B/C provider number Y916Z. Our tax identification number,
59-2641731 will be needed when obtaining a Humana HMO or Gold
Plus referral. It’s that easy!
Dade County Public Schools has kept United Healthcare: benefits
are as follows:
$20.00 co/pay and 40 visits per year
$15.00 co/pay and 20 visits per year
Aetna HMO or POS still DO NOT require an authorization number
for outpatient physical therapy
ONLY a prescription from the specialist or PCP.
AVMED and JMH
(BEECHSTREET) ONLY- require a prescription. Evaluations are
automatically approved for JMH patients then WE request
additional authorization.
